1. Please fill out the form in its entirety.

2. Please attach ONLY 1-3 images that you are comfortable with using, along with any inspiration that you have for your cover design.

3. *Date You Need This By* - I will do my best to get this to you by the desired date, BUT priority submissions will be the RUSH designs. If you'd like to solidify your date, please submit as a RUSH order. Otherwise designs are completed on a first come first serve basis.

4. ALL Cover Photo Submissions come with two included rounds of edits once the first draft has been sent.

5. Images are built on Photoshop only and are not editable after approval. You will not be sent the .PSD file, only PNG or JPG.
